Our residential programs are tailored to each individual's unique wants, strengths, abilities, and treatment plan. From meal choices to activities, we honor personal preferences. Access to hiking, games, employment, and community events ensures a rich experience and are accessible to all. Our communal spaces, including laundry, living rooms, dining areas, and kitchens, promote a sense of community. Every participant is encouraged to take part in community events, whether individually or as part of a group, in accordance with their personalized plan. Support staff empower individuals make choices in engaging in community activities to ensure members have real life experiences. Bridge Support Services believes community engagement and building relationships enriches people and empower to live their best life. Transportation, including agency and personal vehicles, is provided as needed or based on priority needs. Support staff will assist members to accessing public transportation and or in arranging transportation for planned activities. Medical appointments, work, and community events are facilitated by our support team as well. We ensure every individual has access to transportation for work, home, and community engagements. Privacy is ensured with lockable bedrooms and bathrooms. A low staff-to-client ratio allows for flexible scheduling. Safety is prioritized with locking doors to the outside, accessible to all residents. Bridge Support Services believes that all individuals are capable of self-determination and self-direction in their lives. Bridge Services' commitment to encourage peer-to-peer learning, individual rights discussions support by informing individuals and their families of self-advocacy groups.

Case Management Services
Community Case Managers help individuals access employment, education, housing, social and other supports, and necessary medical services. They work with individuals, families, service providers, and other agencies to create a Person-Centered Plan (PCP) based on a comprehensive assessment of each
individual’s needs. In addition, Case Managers assist individuals and parents or guardians in navigating the service system.
Community Case Managers are responsible for:
Assessing needs and completing a comprehensive assessment
Developing a Person-Centered Plan (PCP)
Offering referrals for services
Coordinating services
Monitoring to assess an individual’s safety, well-being, and stability, as well as monitoring the progress toward the goals identified in their PCP.
Protect the individual rights
Assistance with arranging accessible transportations
Comprehensive transition planning for people leaving school systems
Coordination of supported vocational, residential and community support options in all phases
Shared Living Provider Eligibility
Must be 21+
Must be in Good Physical and Mental Health
May be an Individual, Couple or Family
Must have Creativity and Flexibility to Support Individuality and Growth
Must Possess a Willingness to Work with Team Members and Service Agencies
Experience working with individuals with developmental disabilities preferred, but not required
Providers must meet the requirements of OADS as follows:Successfully pass background checks as required by the MaineCare Benefits Manual. All other adult household members (18 years or older) who live full or part-time in the home and/or who will provide support and/or transportation to the individual must also pass background checks.
Successfully pass a home safety inspection conducted by an Administrative Oversight Agency to assure the home meets all health and safety environmental standards according to the MaineCare Benefits Manual and DHHS-OADS
Ensure that their home meets local housing codes. The Provider will submit a copy of the initial home inspection findings to the Oversight Agency.
Verify he/she has time to provide daily services/supports which meet the needs of the individual and is willing to work towards the goals the team has identified in the Person-Centered Plan.
Have a high school diploma or GED and a valid Maine driver’s license.

The Shared Living Program by Bridge Support Services is designed for adults aged 18+ with developmental disabilities who require family-based support. It offers an alternative to group home living by matching individuals with provider families, fostering relationships and community inclusion. Shared Living Providers open their homes and, as part of a team, support in areas like education, employment, recreation, family inclusion, health, and independent living skills. The team may include family, guardians, case managers, and other service providers. Shared Living Providers receive a stipend, training, supervision, and around-the-clock emergency support.
Bridge Support Services Shared Living Program is designed to provide family, home-based services to adults age 18+ with developmental disabilities who are not able to live independently.
An alternative to group home living, adults are matched with provider families who provide stable support systems and natural family environments that foster relationship building and community inclusion
Shared Living Providers help adults with disabilities lead fulfilling and active lives. Shared Living Providers open their homes to adults with developmental disabilities.
They are part of a team that helps in the areas of education, employment, recreation, family inclusion, health/mental health, and independent living skills.
Other team members may include the adult’s family or guardian, case manager, other service providers and Bridge Support Services Shared Living staff.
Support for Shared Living Providers
Shared Living Providers are Independent Contractors who Receive a Generous Stipend, Training,
Supervision and 24/7 Emergency On-Call Support.
